As PP said, if the contributions are deductible in your state, and you contributed to the plan that qualifies for your state's deduction, then you answer "Yes" when TT asks if you have any qualifying 529 contributions and enter the amount.
There's no specific tax form issued from the 529 to you unless you received a distribution, so you just have to enter the amount you contributed that year based on your records. I always put a copy of my year end statement with my tax file so I would have it on hand in the event of any questions from the state taxation authority.
