If she is 65 years old or older, she should now be on Medicare Part A & B. I don't believe Obamacare covers these plans. She probably has an altogether different plan today than she had in the 90's. You need to check with her present plans to see if she is covered.

Isn't the lifetime limit per insurance plan? If she accrued those expenses under a different plan, does it matter? Also, is she on Medicare?

The link above provides information on lifetime limits. The prohibition on this for essential health benefits (which include substance abuse services) ends for plans starting in 2014. I think the best thing to do right now is for her to contact her current insurance directly to see what they cover now. Good luck!
