If you have the license number you can look it up online and print out verification of the license.

You need to submit the entire report that was given to you by the dr. Also you should have a photocopy of her license. If YOU want to interprete the numbers then you can submit a seperate letter from yourself. That would count against the pages you can submit.

Yes, you need to provide a copy of the license and yes, you submit the entire report (actual test results and the entire narrative).
