OP the diagnosis or lack thereof is not your business. You will sound nosy and unprofessional to even mention that to your supervisor. What you need to do is come up with concrete examples of the specific behaviors that affect you and make your job difficult, and ask your supervisor for assistance in dealing with those. If it doesn't directly affect your job, don't mention it. So you would say something like, "Three times in the past month, Hilda has told me she would have these reports done. Here are the e-mails in which she agreed. Then when I asked her about it, she said she had no idea what I was talking about. I was not able to complete my project without the reports." Be concrete, be specific, and leave the personal stuff OUT of it.
