Yes, some percentage of people that have had flu shots will likely die from the flu. However, that is not a useful fact, and nothing should be inferred from it outside of a broader context.

If the child is allowed to dehydrate and medical attention is not sought, yes.
This why children in developing countries die from things like measles, mumps, etc. It's not the primary illness that kills them. It's the secondary effects and it's mostly dehydration because there is no adequate availability of clean water to use for rehydration.
