Box 15 is the State and State EIN for the employer. In your situation, this would be DC. You do not need a MD EIN.
Are you actually withholding local taxes for her? Employers of household employees normally only withhold FICA and/or Federal taxes. If however you have generously opted to withhold state and local taxes, those need to be calculated based on the employee's state/locality of residence. So you would list DC and your DC EIN in Box 15, but boxes 16-20 would all be based on MD and the jurisdiction of residence of your employee. Sorry, I see now that you did state that you only withhold FICA. Because of that, boxes 16-20 would be left blank.
