Not anymore. Last spring I applied to a handful of jobs that did not specify salary. Two jobs contacted me for interviews and in the email said something like “at this stage we’d like to disclose that the range is insulting to infuriating, do you want to move forward?” I did not.

+1. If no salary is listed you can generally assume it will be underpaid.

No. From experience, it's never competitive. They're not sharing it for a reason. I had two interviews with a well-known tech company that didn't share and I thought that would be the exception, but it wasn't.

Yes. At least in my field, if they are interested the recruiter (internal) will contact you as the first step and you can discuss money the. So you have at least a range very early on in the hiring process.
