Just work backwards from appointment time, aim to arrive Baltimore Penn 45 minutes before your appointment. It's around 10 minutes to the hospital from Baltimore Penn.

Silver Spring to JHU campus is about 45 minutes.
Taking public transport takes at least twice as long. Driving to JHU and then parking at Penn station is a much more logical choice.
Baltimore around the train station is sketchy to the max. I would but want to leave my car there or walk through a nearby parking lot.
I would take public transportation , but maybe a cab from the hospital to the train station (because I am not familiar with the shuttle).
It's right here, just to the left of the main station entrance/exit, you walk up to the road, don't have to cross the street. You can see the Shuttle sign in the Street View.

That's the southbound direction which heads to the hospital. On the return it drops off on the other side of the station, right on Charles Street, again no need to cross the street, just walk right down to the main entrance/exit.
It's this shuttle that OP would want.
