Where are you located. Most GP vets should be able to do a skin cytology, look for fleas , do a skin scrape , etc to help identify the cause of the scratching. Treat bacterial/yeast infection. Get an ecollar to prevent further self trauma, use appropriate topicals and get on systemic meds if needed.

Bradley Hills Animal hospital is good. They will likely swab and prescribe steroids.

Anyone recommend a vet who can help with dog who is scratching herself and licking paws to the quick. Tried changing food - didn’t work. Need a good vet. Thanks
