There is a basic letter that an accountant can fill out as a third party so I do not have to show my w2s and other private info to the company in am investing in.

I am the op. Just circling back on this. They want accredited investors. Maybe I do not need the form but I was advised that we should have the letter ready to go in case we are asked.

I am the pp. I have dozens of private placement 506(c) investments and I’ve never had to have it verified. Maybe the rules changed. I’ve always just self certified.

The manager of the investment should indeed supply the form.

To the prior poster, this must be a 506(c) offering. The investor is asked to have a verifier (under the rule, must be an accountant, lawyer, broker, or RIA) independently certify as to investor’s AI status.

Huh?

Why would your accountant need an accredited investor form? And why would your accountant fill it out?

When you make an investment that requires an accredited investor the firm seeking the investment provides you the form and you just check the applicable box and sign.

"They" should have the form that their advisors find acceptable. It is their liability so they are the ones who should supply what they feel is sufficient.

And yes, I have been an advisor to "them". I would never take an investor's own form.

Hi. Does anyone have a simple accredited investor form that our accountant can fill out to meet SEC requirements? They are asking if we have the form. There are many online but just need a simple one that accountant can fill out for us.
