Her reaction may not have had anything to do with you. I think what she's worried about is that the responses are not anonymous and anything she says can be held against her. I had similar questionnaires at my work and worried about the same and always tempered my responses. It's always possible to see who is answering even if it means that IT needs to be asked for the IP address. Someone who aggregates the information has to be serious about confidentiality. I would not want to participate in a 360 on either side of it.
I guess but the 360 review is not place to blast people.
