You want a PT. A good one can document this so that insurance will cover it.

You need a personal trainer that alsonhas a background and recent experience with Pt. This is not unusual, nor hard to find. I would ask around. I would not work with a trainer (esp at your age and with your issues) that does not have less that 15 years of experience and a nice list of clients/refs.

I have a few muscles that are bizarrely weak and messed up, partly (I think) because I had some pinched nerve roots that were making them work poorly. The pinched nerves have improved, but I need to strengthen those specific muscles (left glute medius, something in my left shoulder) before I can comfortably perform a lot of the movements that are involved in standard exercise routines. What kind of specialist do I want to consult in order to target these weak muscles and modify my other exercise as necessary to support them, a physical therapist or a personal trainer? I think it's very likely that the weakness is exacerbated by tightness or overuse in other muscles that have had to compensate for the weak muscles and I need someone who knows more about biomechanics than I do to help me figure this out.
