I would assume its a general intake form and that form is used from 1 day-18 years old. Stop trying to find offense in everything and anything.

It's probably just their standard intake form, so it goes to a newborn, and it goes to a 12 year old. Just skip or write TBD. This is not a real problem.

I would laugh very hard. But nothing beats an excellent pediatrician, so I don't care about the rest. My 13 year old's serious auto-immune disease was first suspected by her pediatrician, who took her symptoms seriously and sent her promptly to an excellent specialist.

Focus on what's important, OP.
