In theory, the IEP team can reevaluate placement at any meeting, especially the annual review. In practice, I doubt that happens if everyone agrees the kid is doing well/the nonpublic school is still appropriate. If the LEA does suggest changing placement and you file for due process, you have stay put rights (but only if you file -- mere disagreement isn't enough).

My kid has been at a NPP for four or five years. There has never been a formal reapproval process. Things have been going fine and once that happens my impression is that people are reluctant to rock the boat/have more meetings.

Approved for non-public placement. How often does the school district need to “reapprove” funding?

My kid has been bounced around to 4 different schools in less than 2 years…this would be number 5. We’d like some consistency for a few years…
