The big question is can he afford to hire help and private pay. If yes, then you can hire a local social worker to coordinate care (IL facility should be able to recommend) or you can arrange it from afar. Get people in from 7-11 to get him up, showered, and in a chair and help with exercises, then again in the evening from 4-8 to do dinner and bedtime routine. It will cost about $6k a month (I think) but if he wants to be home, this is the option. If funds are limited, rehab is the choice.

Having been in your position, I want to point out that this is not your responsibility. If he is of sound mind, it is on him. All you can do is make suggestions, do research and offer solutions. Sometime it takes dire consequences to snap people out of being either obstinate or in denial. Wringing your hands from afar is just going to drive you crazy and probably won't help him anyway.

Yes, he’s at home. I don’t know if the docs or he decided he didn’t need to go to the rehab center. I told him repeatedly that going there was the best to make sure he was able to do (relearn) regular things first was the best option. He said he was doing that up until a few days before the surgery. He kept saying people tell me they could walk within a day or two. (Me: what they don’t tell you is that they have help at home to do the little things!!!!) what a mess.


I'm shocked the hospital discharged him without a plan in place. You usually have to fight to keep them out of rehab. We had to argue to send my mom directly home (shoulder replacement so easier) and we had 24/7 care set up for her. They still put up an argument she should go to rehab.

Home care services can provide what you need. As PP noted it's around $30/hr with a set minimum # of hours.


They should not have discharged him-I agree. He should have gone to rehabilitation. Now I would just hire a case manager and aides and make sure the case manager has a plan for backup if people no-show.


Could you elaborate on what a case manager is and how to find one?

Thanks everyone. Yes, I’m shocked that

a) he was cleared to have the surgery. He’d been hospitalized for 2 weeks for water on the heart. He stopped taking meds to prevent that bc it makes him urinate too frequently and he was having issues changing his adult diapers. PCP said she would not clear him even after they got things straightened out. Then two weeks after his hospital stint she approved it!

b) the doc did not put him in rehab for two weeks and that my uncle changed his mind again about going to the full time rehab facility. he wanted to do therapy at his independent living complex.I told him that takes more coordination than he has wanted to do. He’s worn out the good will of people around him (former neighbors and some hired help) since his wife died 6 months ago. They’d been living in a condo.

As a preventative measure attempt, I’ve left a message today with his PCP that any future therapy plans must factor for the fact that he has very limited to no help at home (or in state). So if on the fence between two plans, and one is rehab facility and the other is home, rehab needs to be first choice.

In the 2 days since his dis charge from the hip surgery, his independent living facility has had to call EMS twice for lift assist. The IL people have called me, and I tell them cannot do anything about his bad choices except to encourage him to make better ones. He is still mentally competent just stubborn. He’s been evaluated in December and before hip surgery by docs. I agree with them that he’s competent but that is not as reassuring as you’d expect.

It's unclear if he is already home or still in the hospital. If still in the hospital they won't discharge him to go home unless he has the people and set up to take care of him.

Rehab also won't discharge him if he isn't able to care for himself and doesn't already have help set up.

Hospital is required to have a discharge plan. Might be to rehab center but then that has a discharge plan. They have lists of home PT, OT providers to call for coverage.
Needs more than "help getting out of bed." I sure did after TKR. Needs PT to build strength and balance and OT to do tasks of living.

My mom is in this situation. She has a home health aide from Visiting Angels but chose to go to rehab from the hospital so it would be covered under Medicare rather than going from PT hours to FT hours.

If he has a lot of mobility issues his home may not be the best set up initially, may need modifications to shower, toilet etc.

Rehab offers PT to rebuild strength and is covered. It's the best option and can really support ongoing independence. If he goes home will all be out of pocket and may not rebuild strength.

He was supposed to go to rehab facility. Then he changes his mind several times. I have legal authority only if doctors declare him incompetent which is currently is not — just making bad decisions. I will call his PCP.
Alternatively, you can investigate getting him into rehab for his recovery. This is covered by Medicare but needs to be arranged by doctor/hospital.

Out of state relative (SE Michigan) has no children or anyone ro assist him. He recently had hip surgery and needs help in/out of bed during recovery period. Is this something a home health aid provides? Medicare?
