Interesting! I just fill out a FSA form online with dates of service, the EIN, and upload the invoice on my FSA processor’s website.
Looking at my invoice, it’s actually so strange. It doesn’t say what I’ve paid for (no description at all), no dates of service, nothing. Just shows I paid the Arc some money. Honestly I can see why my FSA processor denied the claim. This could be for anything. Our prior daycare’s invoices were always clear that it was tuition paid for such and such a month, had my kids’ names on it, etc. Hopefully Karasik can give me something that shows I paid childcare tuition for January.
As long as Karasik accounting signs your form you should be good. I don’t attach any copies of the invoice to the FSA claim form that I upload.
