Attendance matters:
In order for an absence, late arrival or early departure to be marked excused we need the note from the doctor. Please, remember to provide the doctor note for all appointments. We need these notes within 3 days of returning to school

Oh hell no, they don't need to know what appointment it was. As PP above said, remind them of the rule. And so what if it's an unexcused absence? It doesn't matter.
