For the PTO week, each parent would take a day with the kids, covering 4 week days. Both families had local grandparents who helped out with Friday. It worked for us.
We also agreed with the nanny in advance that one week of PTO would always fall between Xmas and New Years.
Crappy arrangement for the nanny. Most families give a Christmas bonus with extra days off around the holidays and don't have that count against the nanny's PTO.
