DP. The mall might be fine, or it might not. Is there a children’s area at the mall where your child can climb, run around and make friends? Or is your child sitting in a stroller for hours while your nanny tries on dresses at H&M? (Even the children’s areas at malls should be visited sparingly. They are FULL of germs).
Library is good. Nature walks. Heck we would even go to PetsMart and look at fish. What about a toddler tumbling program that meets 1x per week? There are also toddler music programs like Kindermusic. These typically meet during the day so perfect for your nanny if y’all can work around nap time. Most children’s museums have a toddler area - your kiddo is not too young for those places. Start checking them out. Many offer memberships so nanny and baby can go as many times as they want.
Also you mention that they are eating out everyday. Again, might be fine, but I’d want confirmation that they aren’t eating fast food everyday.
Lastly I completely understand why your nanny would want to get out of the house - more interesting for her, no cleanup, etc. But kids that age also like super simple things like finger painting or playdoh that can be done at home. Not every day has to be a big adventure.

8:45 nanny and DS leave. Mornings are spent at a playground, indoor gym at the recreation center, story time at the library, or music class
12ish nanny and DS come home, she feeds him lunch
1-3 DS naps. Nanny cleans up lunch, has her own lunch, and takes a break.
3 DS has snack
3:30 nanny and DS head out again - playground or library depending on weather
5:30 return home
She coordinates with other nannies in our neighborhood to meet up at different places. Sometimes they walk, sometimes they drive.
