Yes, I don't understand what happened. Were they in a car together at a convenience store? Or some kind of military vehicle? A regular car or SUV, even idling outdoors for hours, will not build up CO. Maybe if the exhaust pipe had been blocked somehow? Or maybe they were parked in a poorly-ventilated garage, or were in a military vehicle with different emissions?

No. A quick blood test for different forms of hemoglobin will tell you - methemoglobin and carboxyhemoglobin. Also, classically, blood will have a cherry red color, which is not diagnostic, but is a clue.

I would think it would show up in the bloodwork of an autopsy. Where it might go undiscovered is if no autopsy is done. I think they are pretty rare these days for anything that appears to be a natural death.

I wouldn’t think so, if they do an autopsy. What else would they say is the cause of death? Just make up something and be like “yeah looks like an aortic aneurysm!”
