This happens to me if I drink water before working out. Especially cold water.

Ok, so this is probably unlikely, but I suffered from the same problem for years. I tried to run, but every higher-intensity workout resulted in what felt like period cramps. My doctors had no explanation and didn't seem concerned about finding one. Finally, when I started trying to get pregnant, I had an ultrasound that revealed a uterine polyp. It was benign, as most are. Once it was removed, I never had the problem again. You might want to ask your gyn. or do some research to see if you have any other symptoms suggesting a polyp. Interesting. How is this diagnosed? I'm up to date with my routine medical care. (And done having kids, fwiw)
