They're not allowed to explain the section or let the kid ask questions beyond the standard script and practice questions for that section. They always stop after a specified number of incorrect answers.
My kid didn't understand one of the optional sections and scored in the 5th percentile. The examiner flat out said that he could tell that my kid didn't understand the instructions. His FSIQ was 140+. They still included the horrible score in the score report. That's generally what they do.
