Has he lost weight recently or always been thin and underweight? If this is his normal, then I might give a high quality multivitamin but I wouldn't feel it's necessary. Fwiw, both of my kids are underweight, always have been (and DH and I both were as children but not anymore lol). I give them a multivitamin sometimes but don't stress when we are out.
