The entire rental period up to 30 days costs 19.99 or 24.99, depending on the coverage level you choose. I generally don't use it domestically, but always for international car rentals. You have to call amex and ask them to add premium car rental coverage to your card.
The weird thing about it is, amex won't actually charge your card until you return the car, but I've gone back and forth with the help desk on this and they say they will pay out if there is an accident, as long as your card is registered with premium coverage and you use your card to secure the rental.
