Is this an admission that diet and exercise alone don't work?
diet and exercise work...short term..... The drugs make adherence to diet (and possibly exercise) much, much easier, mostly by suppressing cravings and appetite, but there is an additional benefit in increasing insulin sensitivity, which both helps with cravings and impacts fat storage and metabolism (though the primary mechanism of action here is appetite suppression).
the billion dollar question is whether people who lose weight on the drugs and learn how to eat better are able to go off the drugs ever. So far, most people gain back some of the weight, but with a consistent program of exercise and better nutrition its possible they can maintain some loss w/o drugs. we will see.
