You are residents, or fellows? or some combination of both? Yes, for now the cost of nanny + daycare ("school") seems insurmountable. But be assured that these lean years where you have to pay $$$ childcare costs while not making big-A$$ bucks are short.
Soon you will be killing it financially and you'll make up time and be able to accelerate retirement and education and down payment savings
Until then .. do not park your TWO year old in a corporate daycare for 12+ hours a day. She needs to be in her own bed and with individualized attention during some of her waking hours. If not from you, then from a woman you hire to be 1:1 with her
This is good advice if you can swing it. It doesn’t need to be you or your spouse but it is best if care is mostly a consistent and warm adult. -also a two physician family
