That last one would be the most important one in your situation. An adult will sit with the kid and verify they're answering the entire question.
Others I've seen in MCPS are:
Break Down Assignments Into Smaller Units
Have Student Repeat and/or Paraphrase Information
Monitor Independent Work
Some teachers use Sticky Notes to provide checklists to their students. Older students can be transitioned to making their own sticky notes.
