OP, I've looked at a lot on the eastern shore of Maryland and in lower Delaware.
The newest facility is not necessarily the best. You are really looking at staffing.
Talk to workers in the parking lot as you are walking in. Talk to workers throughout the facility. Ask how long they have been there etc.
Also ask about latex glove availability. I've had a couple of private duty CNA's work for us in the home who prior to Covid worked facilities and were limited to either one pair of gloves per week or one pair of gloves per day. These were at privately owned facilities (owned by one person.)

OP,
Some nursing homes will allow you to hire outside CNA's. Some will not. Make sure to ask that question. If you hire an outside CNA ask what the person is allowed to do. Some only allow "sitting."
If resident is wheelchair bound I'd recommend a CNA (not an unskilled sitter.) CNA will be able to do safe showers, safe transfers etc.

I investigated this with care facilities, and it was always an extra, private add-on that you contract for separately. Does not seem like a sustainable business model to staff at that level as the facility would be prohibitively expensive for 99%.

One person I knew hired “babysitters” rather than CNA’s, but the job was limited to companionship, observation and making sure the person didn’t fall out of bed.

Yes you will have to hire additional aides to come in the nursing home if you want 24/7 in the room at all times.

Sounds like they need a nursing home at this stage and not assisted living. Focus on finding the best nursing home. Majority of assisted living require a certain level of health/mobility before they allow them to move in. Your elderly loved one is past that unfortunately.

For an elderly loved one who is almost entirely wheelchair bound and a major fall risk, I am looking for a type of local facility that would provide him with a private room and 24/7 individual, dedicated, one-on-one care. Basically a place where he would literally have a qualified staffer at his side around the clock, and also where he would receive regular physician visits and medication review/refill. If such a place exists, I don't know how to find it. Internet searches for terms such as "exclusive nursing home" return hits for luxury-appointed, uber-decorated facilities. Small, residential care facilities and assisted living facilities are not geared to the level of care required.
Or is this a matter of locating the best nursing home in the area and augmenting with our own hired aides/caretakers?
