It’s actually not all on you. Weight is complex and multivariate, but the first thing you need to lose is the myth that this is entirely on you. I strongly suggest therapy, but only with a therapist that has a good understanding of the science of obesity.

I would not go straight to weight loss drugs. First, get a full workup including bloodwork and an EKG. I had luck with Dr. Mansukhani. She doesn't take insurance, but will give you the paperwork so that you can submit for it.

Do you live in the DC area? I was you three years ago and went through the Diabetes Prevention program at Holy Cross in Silver Spring. It is totally free! And it changed my life for the better. I was 190 and now I am 142.

Also, read the Beck Diet Solution. It gave me several new tools to use to adopt better habits, especially coping ones that don't involve food.

Dr. Eric Thorn’s cardiac practice at Virginia Hospital Center is pretty close, focused on a keto diet. It doesn’t have psychological counseling but you can meet every other week with his nurse Mary Lou who is like a behavioral coach and nutritionist in one.

Like Calibrate? Don't do that. The extra fee per month is a waste of money. OP, I cannot help you with doctor names. I've spent the past year trying to get help for the 70 lbs I put on during the spring of 2020 that I have not been able to budge. My primary care provider was nice and sympathetic, but not helpful. She recommended a few different places, including a nutritional counseling center and a health psychology center, both of which focus on weight disorders. I did not find them helpful. I was finally able to get a prescription for Ozempic last month, so while that will eventually suppress my appetite, I am still looking for help getting the other pieces (mental, emotional, movement, etc.) into place. If she has insurance, she needs to use that. Someone who is 100lbs overweight and with A1C numbers in the diabetic range needs to be prescribe a med like Ozempic.

So make an appointment with one. If you have a PPO go straight to an endocrinologist.

Good for you for reaching out OP. Your situation is medically complex, and you are taking the right first steps. A couple of points:

DCUM has a large contingent of fat-shamers who are grotesquely ignorant of the science of obesity. In general I don’t think DCUM is a good source of advice for managing and treating morbid obesity. People are too science-ignorant here. I would consider looking for various communities online that are moderated and designed for MO and SMO people.

You hear horror stories about bariatric surgery for complex reasons but it is actually generally the most successful mechanism of permanent weight loss. That may change with the development of GLP-1 agonist drugs but bariatric surgery has a long history of good success. Also, they aren’t just restrictive surgeries. I would work to separate fact versus fiction with respect to surgery before you make a decision against it. Bariatric surgery with reputable surgeons with proper monitoring has generally good outcomes.

The development of drugs like semiglutide for weight control is exciting. Please find a doctor that can talk competently about this.

I would start with an endocrinologist who has experience with morbid obesity. Do not go to any doctor that gives a “diet and exercise” only advice; that’s medically ignorant and will just lead you to regain. You need a comprehensive blood panel, close monitoring, and good discussion of medical options. There are a lot of doctors who know literally nothing about obesity so you have to be careful. If one doctor seems ignorant, try another.
