I assume tip, gratuity and service fee are all tip that goes to the server. Have to look for he thread also where 'most people seem to disagree'. It's hard to find workers and we had one good one quit because of foreigners not tipping her. When I told her that this happens often with foreigners, she said 'I don't care' and she is leaving us.

I think it goes to restaurant not server. Would clarify at restaurant. You can always email if that’s too exposed feeling or you are energetically confrontational.

Usually it states it somewhere, the menu, the app, the website if it goes towards wages or not. If it is 15%+ and does go towards wages, I don't tip an additional amount. If I paid cash, wouldn't ask for change if less than two dollars though.
