You need to bring several items including two recent utility bills, your own driver's license, and your child's birth certificate. We brought our child's passport because that was listed as an acceptable form of ID on the DMV website and were turned away-they said that we needed her birth certificate (we are still convinced they are wrong and that they should have accepted the passport but be safe and bring both).
Good point—I initially forgot the documentation of my proof of residence (which is what allows the parent to vouch for the child). A lot of things work here beyond utility bills; we ended up using bank statements, which are generally easy to print off of bank websites.
We brought birth certificate and passport, and they asked for birth certificate specifically during the application process.
