We recently had an experience at a tournament with a ref who was completely inappropriate. He stopped the game to comment on a parent's attire. During halftime he repeatedly came over and made comments about political conspiracy theories and referenced the Penn State scandal (one of the parents was wearing a Penn State hat). I get that refs are in short supply, but they really seem to be scraping the bottom of the barrel. Especially with the U-littles.

You can notify your league with the specific game and they can notify the ref. If your league is just hiring refs from a state group, then nothing much will come of it. Larger clubs have their own training, and will instruct refs for improvement.

Only once, but we did it through our coach. It was after a game where multiple girls got seriously injured due to uncalled fouls from the other team.

As a coach I complained once about a ref who did not know the laws. Gave very specific details about what he said about his calls and what the laws actually say. I passed it up the chain through my club so it went to the assignor and I actually heard back from the assignor and an apology directly from the referee.
In general, there is a huge shortage of qualified referees out there and unless something is really egregious it will get ignored. Your best bet if you want better refs is to get a whole group together, take the course and get certified, and get out there and do it yourself. Not saying that to be condescending, it's just reality that there aren't enough good refs.
This is the issue right there. Big shortage of refs so it would have to be something crazy for the ref to be removed (one who made racists comments was removed a few months ago but that’s the only one I know of).

Just an FYI. There is no ref supervisors. There are many assignors. I can imagine that if the assignor hit enough grief about one ref, that assignor may choose not to work with that ref but there is such a shortage, that’s doubtful. Plus, there are plenty of other assignors to get work from.

Our coach escalated to club's TD who contacted the supervisor for the ref. But this was a case of a ref who didn't know the rules and was making strange random calls. I think the result was just additional training.

I've made some inappropriate comments to bad refs in the past - but that's about it.

Agreed, unless your coach or team manager files a formal complaint, nothing you can do about it.

Unless the ref commenting on something grossly inappropriate and not soccer related, like “hey nice ass”, it is definitely not the parents responsibility or right to make any sort of formal complaint.
