To me, it does look like some element of ATC error.  It clearly was not communicating to both the helicopter and the airplane that they were headed straight for each other and need to take action.  [/quote]

It seems this was allowed to happen often but no crashes happened until last night. The rules for going around and flying below will need to change.[/quote]
