He thinks his wife is a lazy slob because she's gained weight (he clearly doesn't even know how much) when she's got three very little kids at home by herself. He's not worried about her health, he's not asking how he can support her eating healthier and getting exercise, he's asking how to tell her that he no longer finds her attractive and thinks her weight gain says bad things about her character. His only solution is that she should get up even earlier to exercise at home. He's not offering to watch the kids so she can exercise, he rejects the idea that he should help with meal prep.
