eh, parents are never "off duty".  For all you know this could have happened first thing in the morning while the mom was in the shower getting ready for the day.  When my kids were 3 they used to lay on the bed in the master bedroom watching cartoons while I got ready in the bathroom.  It was our normal routine and they, thankfully, never varied from it.

I followed my kids around like a hawk and I am also having a hard time understanding exactly what happened but that isn't because I'm blaming the parents for this tragedy.  Obviously, something went terribly wrong and out of plan that day. [/quote]
