It really bothered me to be instructed to NOT keep a copy of the form after I submit it. You can be sure as hell I'm keeping a copy. I might keep it until the day I die, just in case someone ever accuses me of not following through on suspected abuse. There is no way that I'd ever knowingly or with reasonable suspicion allow a child to be abused. (MCPS teacher and CSA survivor)[/quote] I would absolutely do the same. Why would you not keep a copy?
