You should see a page prior to the "upload here" page that asks for name, etc. I had to clear the cache in my browser to see the initial page. Also, the information they want is actually *different* from what the fields are labeled (totally not user-friendly, but that's an aside...). Read the instructions under online submission on this page: https://www.apsva.us/registering-your-child/hacp/ They note everything they want you to put in the fields in the instructions and provide additional info.
