Anybody know of something like this?[/quote] No, but if you have a spouse or close friend or relative whom you can trust with your life (literally), you can have a conversation about this ahead of time and ensure that you go out on your terms. It's up to you to procure the supplies, which isn't hard if you do some research, but it will be up to them to put everything within your reach and make sure you actually do it when you get to that point.
