Here's another: the 340B drug subsidy program. Providers purchase drugs at heavily discounted prices but get full reimbursement from Medicare or Medicaid. The idea was that with the extra money they could fund additional healthcare resources in low income areas. Instead many of them take the extra money to build fancier facilities in wealthier areas (with a focus on profit-generating facilities) often while also closing critical programs in the low income areas they serve (like, say, actual emergency rooms).
