We did at two preschools that have the same requirements. We mostly vaccinate but there are a few we do not do. We included the vaccination chart and signed the religious exemption. The exemption just has you sign and does not ask you to specify why or what religious reasons. Not a big deal but we haven't tried public school yet - we will probably do them before then.[/quote]
