"To establish justice, insure domestic tranquility, provide for the common defense, promote the general welfare, and secure the blessings of liberty for ourselves and our posterity. "Promote the general welfare" can absolutely be read to include public health. Actually, I'm not sure how you can say that a government IS promoting the general welfare of its citizens if it does nothing for public health or the environment, or works actively to harm both of them.
