They will call you if they want more info. Type in something like “Jane tested positive for Covid and will be out of school on …. This will be ten days from when her symptoms began (or the positive test). I will contact yi if there are any changes.” That’s almost exactly what I sent to the school for my child and they followed up with a phone call, mostly just asking if he was okay and how he was feeling. Tell the school by email or by a phone call that you are keeping them home ten days. Send your kid back after that. There isn’t a form. The only form I ever had to fill out was when one kid was sent home with symptoms. They gave me a form and I had options and a paper to fill out before returning. When my kids actually had Covid, there was no form and they just needed to stay home the ten days. Someone from the school will contact you for sure if they want more info. [/quote]
