100% this.  I will say that I'm a pediatrician and our practice takes medicaid so I see a wide range of different socioeconomic backgrounds in our patients.  Yesterday I saw a 5 year old child who was 105 pounds.  She was tall, and obese.  I can imagine that she would need to buy a size 12 pants (if not larger, honestly) and hem them.  Or a size 12 shirt and cuff the sleeves.  So yes, it's definitely possible that a 6 year old would need those sizes due to morbid obesity, unfortunately, and I'd just buy them as suggested on the tag!  And if you're correct that this person was being dishonest to share the clothes with someone else- again, what's the big deal?  Maybe a cousin or a neighbor didn't get on the angel tree list in time (you have to apply sooo far in advance for some of those, or have a church or a school add you to the list, so many needy kids don't end up on the lists) and the mom is trying to do a kindness and share.
