Whether they admit it or not, the majority of Holywood actresses smoke to help keep their weight down yet most of them don't end up with visible "cigarette lips" at 53 so there is obviously so.e sort of cosmetic procedure that helps hide the effects. She probably had some buffered down.  Helen Mirren has major cigarette lips but she's still damn beautiful.  At least guys can grow a mustache to hide the cigarette lips. (Some women probably can as well,  but I have a feeling the entertainment industry would frown upon this--pun intended.)

I do agree with PP, Helen needs to add a few pounds.  She looks too sinewy.  She also has a very large forehead, which unfortunately makes her look a little more disproportional.
