I will just gently note that very few news agencies have real people with boots on the ground to do in-person newsgathering. A lot of modern-day news has a single/few sources and is amplified when seemingly reputable agencies start quoting each other, making it seem as if the story has been corroborated through multiple diverse sources. The need to put up a headline and copy drives news agencies to do this because they don't want to look like they are behind the curve. This is what primarily gives rise to fake news stories - stories that turn out to be untrue as additional information is gathered.
