Yes.  Boone & Sons.  They have made a couple of rings for me, and they also deal in antique jewelry.  They have locations downtown, in Chevy Chase and in McLean.  The downtown store is always my first choice.  They can do anything you want and we have bought a lot of stuff from them over the years in addition to my rings, had many things (inlcuding an antique pocket watch) appraised there, and consider them our jeweler of choice.  We have never had a bad experience with any transaction.  One of the rings they made for me was a really big deal for me (the proverbial push present, and it is twice the size of my engagement ring).  In a situation in which I was going to be really picky about the end product, they delivered 100%.

I'll second the Boone & Sons suggestion. I prefer the Chevy Chase location though. But, if you choose to go downtown, check out Tiny Jewel Box while you're there. It's only a couple of blocks up Conn. Ave. from the B&S shop. They hit where the example you posted misses. They do that style rather well, and though it's not my taste, I've seen very good examples at TJB.

Tiny Jewel Box is usually twice as expensive as Boone & Sons.  I find that Boone & Sons will work with you on the price.  Tiny Jewel Box prides themselves on being super exclusive.
