Nobody wants to give bad care. But if you have two docs doing the work of nine, there isn't a lot of leeway to make the subtle calls and take more time, because someone else is going to be crashing down the hallway.

I'm not in the DC area. My colleague, who delivers babies, dealt with a lot of people at about the same time last night. One was the partner of a different patient -- high on something, stripped to his waist, spitting and swearing. The man yelled that he was going to burn the place down. Took a swing at a nurse. Security called police, and the doc stayed with the female patient and her children until the police came. The doc left, but when she called later to check on her patient at home, the man -- not arrested after all -- answered and threatened to kill her.

And she was delivering babies, And she was assessing for things like fetal demise, and sepsis, and all the rest. And, by the way, she's leaving this job soon and going back to her childhood home. I don't know if she'll still be doing this kind of medicine, or even medicine at all. She has young kids herself.

This is just the way it is these days. Remember the real world setting when you sign off on your support for a bill. How is that actually going to work? [/quote]
