Total and complete failure of military leadership and personnel from the top to bottom. The failure is both tactical and strategic. This is not even adding in the incompetence and graft that is US defense contracting. [/quote]

No. This is on top brass. They knew war was illegal and went through with it. They could have refused unlawful orders.
