It's MONOPOLY. 
UHC is big into vertical and lateral integration. DOJ antitrust investigation.

UHC has also been doing pharmacy benefit management services since the 80s. I always have to look up how this works because its a slick scheme, but it sort of amounts to legal kickbacks and increase Rx prices. 
There's also a complicated trick where they can use their setup to actually exceed the 20% cap on insurance profits via transactions among different subsidiaries (profits will not appear to be more than the cap). I don't know to what extent UHC has been able to exploit this, though.
