Well, for all the pregnancies after Carmen, it happened by him giving a sample 10 years ago that created some frozen embryos.

Then Hilaria calls the surrogacy agency and 9 months later the baby is born in one hospital room and brought over to the next room so Hilaria can sit in bed and pretend like she just delivered it herself. This is actually a thing in the surrogate world. The "intended mother" puts on a gown, gets a blood pressure monitor and looking all glammed up with not a drop of sweat, poses for the newborn photos. Apparently it's to promote bonding, just like wearing a moonbump allows them to relate to the surrogate (who is actually pregnant).

What you are referring to how pregnancy typically occurs, well, from what I've seen, that hasn't been going on for a long time between them. Alec lives in a separate apartment next door down the public hallway. There is no chemistry between them, but I think they both know they are stuck with each other.[/quote]
